Get your medical record number
Your medical record number, which appears on your Kaiser Permanente ID card, is your passport to Kaiser Permanente and you will need it when you connect with us in person, online, or over the phone.

If you haven't received your Kaiser Permanente ID card in the mail, or if you misplaced it, you can order an ID card online or contact Member Services.
